Parishioner Tuition Affidavit Forms for 2020-21 (Due January 24)

Each year, the Diocese requires that we formally record and approve all parishioner tuition subsidy requests with a signed approval form.  If you are a parishioner of
St. Anthony parish, please complete the form by clicking the link below and returning the form to the school office by Friday, January 24.  Those who are members of other parishes (without a school) simply need to indicate the name of their parish at the time of registration for 2020-21 (opening at the end of January) and we will submit an internal approval form to your parish on your behalf.
