Each year, the Diocese requires that we formally record and approve all parishioner tuition subsidy requests with a signed approval form. If you are a parishioner of St. Anthony parish, please complete the form by clicking this link and returning the form to the school office by Friday, January 29.
Per Diocesan policy, those who are members of another Diocesan parish with a school do not qualify for a parishioner subsidy for attending St. Anthony School.
Those who are members of another Diocesan parish without a school qualify for a parish subsidy paid to us on your behalf by your parish. However, you do not need to submit this form. Simply indicate the name of your parish at the time of registration for 2020-21 (opening at the end of January) and we will submit an internal approval form to your parish on your behalf.
